the realm and powers of chaos are eternally playing the minds of mortal men and their numbers vary constantly as they draw new people in with promises of great power and also from the great amount that die in the service of the gods of chaos. the realm of chaos has existed, as you rightly point out, since the old ones opened the portal at the top of the world and ever since then it has existed and effected the thoughts and alliegences of all creatures of the world.

the realm of chaos is a vast vast place with many layers, but it IS a savage place. it is a realm of total chaos, where war bands constantly fight each other and there is no structure beyond the internal hierachies of the tribes that dwell inside it's influence, a hierachy purely based on strength and power. great war leaders command their tribes and can only survive by fighting and constantly moving and worshipping the gods in the hope that they will bestow favours upon their tribes men on the great quest to becoming one of the gods chosen or even being elevated to daemonhood.
the only times when the chaos realm ever comes into line and unites is when an annointed is chosen by the gods and after carving a bloody swathe across te realm of chaos, eventually unites the tribes and unleashes them and the power of the gods into the mortal realm.
but the realm of chaos is a place of eternal warfare and is chaos in every sense of the word.
